Where are you located?

Our mailing address is 961 N. McQueen Road Chandler, Arizona 85225. We are located on the Southeast corner of McQueen and Ray Road, directly behind a CVS pharmacy.

What are your office hours?

We are open from 8:00AM to 5:00PM, Monday-Friday. Please note that our office is closed daily for lunch from 12:30PM to 1:30PM.

How can I get a refill on my prescription?

When you need a refill for a medication, please contact your pharmacy first. They may either give you the refill or fax our office a request to obtain authorization for the medication. If the pharmacy is unable to refill the prescription or fax us a request, please call our office and leave a message on our refill line along with the name and phone number of the pharmacy of your choice. Please allow two business days for a refill request to be processed.

Who do I talk to for medical advice after hours or on weekends?

If you are in need of medical advice after hours or on the weekends please call our office and follow the directions on our automated system.

Can I always see the doctor I want?

If you set your appointment in advance you can generally see the doctor of your choice. If you have a sick child who needs to be seen we will try to accommodate your choice of doctor but if that particular doctor is unavailable we will then have them seen with a doctor who is available. Our primary goal is to take care of sick children as quickly as possible.

Who can help me with billing or payment questions?

If you have billing questions please call our outside billing agency at 602-354-4682. If you would like to make a payment you may either mail it into our office or call us and we may take your payment over the phone.

Do you charge a fee for medical records or FMLA forms?

Yes, we charge a $25 fee for medical records that are released to parents for any reason. We also charge a $25 fee for all FMLA forms.

What are the recommended dosages for Motrin and Tylenol?

For dosage information please refer to the charts below. Please note, if you have a child three months old and under with a fever you need to call our office. Please do NOT alternate Tylenol and Motrin unless advised by the Dr’s or Triage Nurse, as the administration of these medications needs careful observation and monitoring.
